Neighborhood House Resource Specialists, Care Coordinators, and Medicaid Case Managers offer a variety of support programs to individuals with disabilities, older adults, and their families. We assist in planning for critical needs, identifying resources, connecting to services, and navigating the long-term care sector. We believe that our community members should be able to remain in the communities they love, regardless of physical or cognitive limitations.
Where to Begin: Information & Resource Referral
Not sure where to find the help that you need? Residents of Seattle and South King County can contact Neighborhood House for information and referrals related to aging and disability services. This can include a one-on-one meeting with a member of our Aging & Disability Services team to create a tailored list of resources that meet your or your family member’s unique needs. Support is available in a number of different languages and in-person meetings can occur at a variety of Neighborhood House sites across Seattle and South King County or in your own home.

Caring for a Loved One: Family Caregiver Support
Neighborhood House knows that helping a loved one to stay in their home through physical or cognitive limitations is no small task, and we coordinate a number of programs and services to try to help you continue to provide this loving support. From resource referrals to caregiver support groups to one-on-one sessions with a member of our Aging & Disability Services team, Neighborhood House aims to provide comprehensive, culturally competent support to the unpaid caregivers of King County. Support is available in a number of different languages and in-person meetings can occur at a variety of Neighborhood House sites across Seattle and South King County or in your own home.

Create Health and Wellness Goals: Health Homes Care Coordination
Some Medicaid recipients in King County qualify for Health Homes, a Medicaid benefit that can help you to meet your health and wellness goals. Health Homes provides a dedicated Care Coordinator to meet with you in your home or at the location of your choice. Your Care Coordinator will partner with you to develop and work toward your goal. Care Coordinators can assist Health Homes participants with any number of goals, from better managing their chronic conditions, to decreasing pain, to adopting a long-awaited pet. Care Coordination services are available in a variety of different languages.

Case Management for In-Homes Caregiving Services: Long Term Care Medicaid Case Management
Medicaid recipients who receive paid, in-home caregiving services also receive comprehensive case management support. Neighborhood House partners with the City of Seattle Aging & Disability Services Department to provide linguistically and culturally competent services to participants whose preferred language is Arabic, Amharic, Farsi, Oromo, Tigrinya, or Ukrainian.

South King County Region Lead: Community Living Connections
Are you a provider looking to connect with other providers that serve older adults, individuals with disabilities, and their families in South King County? Neighborhood House coordinates the South King County Community Living Connections network, which brings together providers from our region for resource sharing, problem solving, and professional networking.
