Youth Case Manager II

Department: Education & Community Services
Reports To: Youth Services Manager
Status: Regular, 40 hours per week, 12 months per year
FLSA: Non-Exempt
Hiring Range: 16 ($19.82 to $21.99 per hour)

The Youth Case Manager II has the primary responsibility to provide intensive case management services to high-school youth and their families, who are re-engaging with schools, in order to improve school performance, build social assets, and create career and educational goals. The Youth Case Manager II will directly provide and/or coordinate a comprehensive menu of services for youth. These services include: determining eligibility, completing a needs assessment and case plan, making referrals and service linkages, counseling, performing home visits, crisis intervention, documentation of services, follow-up services, and evaluation. These services will support and guide youth toward reengagement in school and/or career opportunities.

  • Collaborate with Re-engagement Team on intake and screenings to determine eligibility for referred youth.
  • Collaborate with Re-engagement Team on conducting a needs assessment and develop individualized case plans for each youth.
  • Provide intensive case management services for a caseload of at least 40 youth with a focus on youth not engaged regularly (weekly and monthly).
  • Coordinate on-going services and new referrals with re-engagement team, other school staff, and service providers.
  • Provide information, resources, referrals, and advocacy to link clients to appropriate services in a timely manner.
  • Provide career planning assistance that helps youth set career and education goals, and prepare a written, youth-driven, individualized career training plan that is based on each youth’s career interests.
  • Provide mentoring directly or through referrals to local organizations.
  • Provide follow up services, in partnership with Re-engagement Team that includes support, counseling, monitoring, and crisis intervention in family relationships, peer relationships, communications and self advocacy skills, behavioral issues, and school involvement, and other possible areas that may become barriers.
  • Build and maintain good working relationships and collaboration with Re- engagement Team, school staff and other community service providers.
  • Participate in home visits to provide support, resources, and communication to the youth and their families.
  • Assist with group activities, workshops, trainings, and special events (including summer time events).
  • Record and document all services and complete monthly reports, including school attendance in a timely manner.
  • Support team in compliance with program requirements, such as 1418 requirements on a monthly basis.
  • Work as a member of the Youth & Family Services team and the Neighborhood House Education and Community Services Department.
Minimum Qualifications
  • AA Degree. Relevant experience with youth and families may substitute for formal degree.
  • 3-5 years of experience working with youth and/or families in social work, human service, or school settings, or any combination which provides equivalent preparation.
  • Outstanding interpersonal and relationship building skills and a non-judgmental, positive, problem-solving attitude.
  • Team player who is resourceful, proactive and comfortable working with diverse populations.
  • Ability to work with multiple partners and service providers.
  • Ability to work effectively in a multi-cultural environment in schools, family homes, and with co-workers.
  • Ability to communicate effectively, both orally and in writing, with individuals from a wide range of cultural backgrounds.
  • Experience conducting group activities, trainings, and classes/workshops.
  • Computer literacy, including MS Office.
  • Willingness to comply with established agency policies and performance standards, which may include productivity/personal responsibility, client/customer services, teamwork, and maintaining a professional demeanor.
  • Ability to work primarily during public school hours and an occasional flexible schedule, including some evening and weekend events.
  • Must pass Neighborhood House Background Check Requirements.
  • Must have reliable, independent, transportation for frequent last-minute travel between Seattle worksites.
  • Current First Aid certificate (within 30 days of hire).
  • Current CPR certificate (within 30 days of hire).
  • Child Abuse and Neglect Training (within 2 weeks of hire).

Desired Qualifications
  • Bilingual ability in Spanish with English skills sufficiently strong to communicate in and prepare documents in English.
  • Experience working with immigrant/refugee youth and families.
